The state will no longer be sending immunization compliance forms and reminders to schools via US mail.

Instead, the Department of Health Services (DHS) is making the information available in a booklet that includes all the forms, a flowchart for what to do in fulfilling your duties related to documenting student immunization information, and FAQs for parents to get them to comply with having their child immunized and sharing the documentation with the school.

Private schools should use the DHS booklet, which is available as a PDF, To actually follow the immunization law. It provides all the information schools formerly received via the mail from DHS at the beginning of the school year.

Private schools are not subject to FERPA, so the handbook has a special form (see pp. 31-32) to report parent non-compliance to the Wisconsin District Attorney.
