Schools submit the 3rd Friday in September count report in OAS as well as applications for enrolled choice students not submitted by September 1st.  WPCP schools should also include address verification for incoming Choice students.  If any choice students have moved, WPCP schools must have received residency documentation with new address by this date.

To correct OAS, the school should email privateschoolchoice@dpi.wi.gov with the following:  student's current name in OAS, student's current birth date in OAS, the information that needs to be changed and, if the change is to the student's legal name or date of birth, proof of the correct name or date of birth.
